PhD applications are no longer being accepted for Fall 2012. Please ensure to complete your secondary application as soon as possible. If you have questions, please contact Jennifer at jkuennin@health.usf.edu. Thank you.

Both Primary and Secondary applications are now online! Applicants will need to complete the Primary Application first before beginning the Secondary Application (SAMS). Please see instructions below on how to complete both applications.


 

Primary Application Procedures


  • To access the Graduate Application for Admissions (click here). Please make sure to select "Medical Sciences"

  • Application Fee :$30

  • GRE Scores (General Test Only). USF’s GRE Test Code is 5828

  • GRE test scores earned within five (5) years of the desired term of entry

  • Original Transcripts (U.S. Only)

  • Course-by-course TRANSCRIPT EVALUATION to include GPA (International Applicants)

  • Evaluations must completed by  AES, JOSEPH SILNEY, or WES

  • TOEFL Test Scores (International Applicants ONLY)

Secondary Application Medical Sciences (SAMS)


  • Complete Secondary Application (SAMS)

  • Completed Science GPA Calculator. Located in SAMS

  • Personal Statement describing interest in PhD studies (1-2 pages). Upload to SAMS

  • Curriculum Vitae or Resume. Upload to SAMS

  • 3 Letters of Recommendation. (Must be sent to program office).
